Welcome to the Chronic Kidney Disease Clinic
Your
nephrologists/kidney physician has asked you to attend the CKD clinic
so you can learn to better manage your kidney disease. Studies have
shown that the information, care, and support given to you at this
clinic will provide you with health benefits.
Who will be involved in your care?
- You
, your family, and those who play an active role in your life. During
each visit you will meet with some or all members of this clinic.
- Clinic Nurse
: She will weigh you and take vital signs. The nurse may perform
certain lab work. Injections may be given if they have been ordered.
She will assist in coordinating your care in the clinic.
- PA Physician assistants (PAs) practice medicine under the supervision of
physicians and surgeons. PAs are formally trained to provide diagnostic,
therapeutic, and preventive health care services, as delegated by a
physician.
Working as members of the health care team, they take medical histories,
examine and treat patients, order and
interpret laboratory tests and x rays, and make diagnoses. PAs record
progress
notes, instruct and counsel patients, and order or carry out therapy. PAs
confers with the supervising physician and other medical professionals as
needed and as required by law.
Nephrologist: A nephrologist will supervise your care and kidney management though he/she may not see you on every visit. |
